If you encounter and kill a Boethiah follower, you will receive a miscellaneous objective to “Investigate Boethiah Cultist”. Search the cultist and take the book off him. You can only receive this quest once you reach level 30, so level up if you havent already or you will NOT recieve the quest upon arriving. If you are level 30, simply discovering the shrine will give you the quest. You will then be prompted to find the Cult of Boethiah.
They arent far off. Follow your quest marker to the cult and a priestess will approach you, give you a Blade of Sacrifice, and tell you that you must bring a follower to the Pillar, activate it with them in the vicinity, and slay them on the pillar. Youre probably not too keen on killing any of your followers that youve gained in your adventures, so it may be best to grab a Hireling to do the dying. Hire one for 500 gold and bring him to the pillar. Place him near it,(you have to order him to touch pillar) activate it, and once he becomes attached, do the deed. Boethiah will possess your now-deceased follower and give you further instruction (if you have also completed the main Dark Brotherhood quest line, you can just use one of the initiates, they dont cost a singe septim). Also, if you just kill the apostles, she will take over one of their bodys and proceed with the quest. (If you choose this route, I advise having Ice form.)
Next youre gonna have to kill every one of Boethiahs apostles, and theres a decent amount of them. Stay back and let them duke it out, then come back in to clean up the rest. Once thats done, Boethiah will appear again and instruct you to take down her previous Champion, take his Ebony Mail, and kill the rest of his bandit companions. You dont actually have to kill them steathily; killing them the old-fashioned way will suffice. Head to Knifepoint Ridge and take them out, and retrieve the mail. Boethiah will contact you one last time once you equip it, and congratulates you. She gives a brief speech and then allows you to keep the armor, which envelopes you in a poison shroud during battle which damages nearby enemies, and also enhances your sneaking abilities. Plus it has a high armor rating. Sweet.
Fix: If you have changed the default controls, reset them to the defaults. This should allow you to give orders to your followers and make them touch the pillar. Fix: Make sure your followers arent essential. Most hirelings and Dark Brootherhood Initiates are essential.
Fix: Just start killing all of the cultists and when you slay the last one, Boethiah will start talking to you and give you the next part of the quest.

I think iv figured out what this bug is about, after you kill the first cultist who comes for you, check that the mission marker to investigate the cultist is on. Do this before you search his corpse and take the book from him otherwise it doesnt recognise that youve taken it, and the mission wont complete.
I am stuck as well, i play through the quest and at the end there is 2 of the old champions followers way out of the map, instead of being at Knifepoint Ridge. Ive tried it with and without mods and even loaded a save from before i read the book. anyone have an idea on how to fix this? Im playing it on xbox one. (edited by A Fandom user)
the are no cultrists for me, literally nowhere. Ive tried different ways to start the quest (reading the book or just sacrificing someone), but not a single npc would spawn around the shrine. I am level 35 so high enough to start the quest, Boethiah instructs me to kill the cultrists but there is just no one there, nog even the priest. How do i get to finish this quest? its literally the last one of the daedric artifacts i still need for that trophy. Please help (edited by A Fandom user)
